Canadian Forces were warned for 20 years about safety gaps before the Wainwright fire

Documents given to chief of the defence staff Gen. Jennie Carignan at the end of 2025 found at least 27 safety regimes that often overlapped, leaving it unclear who was responsible.
Reports and audits from 2003 to 2023 found weaknesses in structure, policies, culture and oversight. The documents said “a fundamental transformation is required”.
